Hi rnrj,

The medicals form I received contained self printed address which shows

LOCAL CLEARANCE UNIT
DIMIA,
GPO BOX 9984
SYDNEY NSW 2001 AUSTRALIA

I wonder where you got the address for the courier which is:

LOCAL CLEARANCE UNIT
DIMIA,
LEVEL 6, 26 LEE STREET
SYDNEY CBD NSW 2000 AUSTRALIA

Are both different? If I send using courier to the former, is it okay?

Rgds

Hi all,

I am a bit late on this thread, but a question had arisen:

I got my medicals via email and the small box "OFFICE USE ONLY"
wasn't filled.

What should I do regarding the FILE NUMBER?? Usually we receive the form
with File # printed in it...

In this case, should I write a cover letter referencing my File Number
and then give it to doctor?

Anyone in the same situation? What did you do?

cheers,

We had ours by email & it says in the instructions:-

"In office use only sections please write your file reference number
and your ICSE request ID"

Thank you Tre,

BTW, did you write both File # and ICSE ID in the same field "case
number"?
What about the other fields (date, visa class)?

George.
