I'm pleased to say that the elusive vaccination supplement required for
AOS has now been obtained, and struck off our list of things to be
worried about. In the end the whole experience was fairly quick, but
very odd and slightly surreal to be honest. It started the moment we
got into the reception area of the hospital.
Us - "Hello, we've come
in for the Vaccination Supplement needed for Adjustment of Status."
Receptionist - "We don't do that here."
Us - "Yes you do."
Receptionist - "Alright, but there will be a fee that you'll need to
pay, it can't be billed to insurance."
Us - "When we called we were
told it could be billed to insurance..."
Receptionist - "oh ok, we'll
do that then."
The Doctor met us at the door to his office (and
remarked on the huge file of documentation we brought with us).
Us -
"Hello, we've come in for the Vaccination Supplement needed for
Adjustment of Status."
Doc - "I'll have to give you the entire medical
again..."
Us - "No, we don't need that."
Doc - "Ok, I won't do that,
I'll just fill out the supplenent shall I?"
Us - "That would be
wonderful, thank you. We'd also like a photocopy for our own records,
is that possible?"
Doc - "I suppose so. You obviously know what you
want so I'm sure that will be alright."
We waited in reception for a
few minutes and then the Doctor came back to us with a completed form
(in a sealed envelope marked "INS") and a photocopy of the form inside
the envelope. This differs from the Vaccination Documentation
Worksheet received at the medical in London in several ways. For a
start, the Supplement is portrait format (tall and narrow) as opposed
to the landscape format (short and wide) of the worksheet. At the top
of the sheet it gives this title -

Beneath this heading is a small
section for the applicant's details, and an array of boxes detailing
the vaccine, date received etc. At the bottom of the sheet is a box
(ticked by the Doctor) which states "Vaccine history comlete for each
vaccine, all requirements met." Finally at the bottom is the Doctor's
signature and the date.
The whole process took about ten minutes, five
of which were spent at reception talking about how to bill the work to
insurance!
So, one more thing taken care of, and so it goes on...
