I like Marnee's fee better, too! We paid $115 here in Phoenix! But I
was also kind of rushed and wanted to get it done that day, so chose
the only civil surgeon near me that was open on a Friday (I think it
was a Friday).

We got the Vaccination Supplement done in San Antonio for $13.

I tell my clients to call around and check with the Doctor to find out
their prices and whether or not they are still authorized by the USCIS
to do this work. I also tell them to ask the Doctor (during the call) if
they have the required forms. If the Doctor doesn't, my recommendation
is to try to find a different Doctor.

I didn't bother asking beforehand how much they were going to charge us for
Marina and Vitaly's vaccination supplements. We had all of the vaccination
records with us... so all he had to do was copy the information onto the
I-693 and sign it. When I went to the window to pay I was expecting to hear,
"Oh...that'll be $300" or some other outrageous amount. But after a few
minutes, the nurse opened the window and said, "Oh, there's no charge."
!!!!!

Marina started to ask them why they weren't charging us anything........ but
luckily I was able to drag her out of the office before they changed their
mind!



Btw, you can download the form yourself from the CDC website. I downloaded
everything---the I-693, the appendix to the supplement (which is what they
fill out in case you haven't finished a particular series of shots because
of the time involved), as well as the civil surgeons instructions for
filling out all of the forms. I took all of this to the appointment with me
just in case the doctor turned out to be one of the clueless ones regarding
the vaccination supplement. It turned out that it wasn't necessary, although
the doctor WAS perhaps a bit clueless. At one point he mentioned that he's
done "quite a few of these in the past few months but had never done one
before that, so this must be some new rule with INS."

This is certainly good advice if you happen to live in a large
metropolitan area. We had a choice of exactly *two* doctors, one of
which died about two weeks before we made the phone calls. :(

Our guy tried to talk us into a new medical, but we went armed with the
page from the USCIS site which explains that K-1s don't need one if
they're filing for AOS within the year after the initial medical.

We also asked the doc to make a copy of the supplement before he sealed
it in the envelope, and after looking at us quizzically and then at the
large binder of information we had brought, he ran off to the copy
machine. :D

The whole process took about a half hour and I believe we were charged
about $45. Their office did submit our charge to my insurance
company, and it was denied. Reason given? Nothing for immigration is
covered. Ah.

Our other choice would have been to drive two and a half hours into
Chicago, and I shudder to think what we would have been charged
there! :)

Hi SG,

I'm going to deduct Sadegh's $115 vaccination from our taxes as either a
medical expense, or something on his business expenses, whichever the
tax guy thinks is more appropriate. Also, I am going to have him try to
deduct the $175 EAD fee in Sadegh's business expenses....it was a
necessary business expense, after all! He can't work without it!

Hi Rene,
Getting our taxes done next Wednesday. I'll let you know what our
accountant says about the EAD, etc. As far as the medical stuff goes, I
do know that you are only allowed to claim a certain percentage of the
medical expenses based on your income, and if you make too much, you
can't claim any. We'll see! And even more off topic---still waiting on
an answer to the alimony deal, too! Once Wed. has come and gone, and we
have some answers, we might want to start a new thread and have people
compare notes. :)

SG,

We've got an appointment for taxes on the 21st. If Sadegh can stay in
town that night, we'll go to it. Otherwise, we might have to wait till
next month :( Starting a new thread for comparing notes on deductions
sounds like fun. :)

I don't know all the ins and outs of the deductions. I just collect
everything all year, total it up at the end, and the tax guy does his
magic. LOL
