Default vaccination supplement for i-485

Hi - My wife came over on a K-1 and now we are filing the 485, AOS.
For form 693, it says she does not need to fill out the whole thing and
only needs to provide the "vaccination supplement". Neither I nor one
of the designated civil surgeons has any idea where to get this
form/supplement. Does anyone know?

Sevros,

One of the forms for the visa medical exam is DS-3025 Vaccination
Documentation Worksheet. This is what a K1 would submit with the I-485
and the examining physician should have given her a copy of it.

If your wife was not given a copy of her DS-3025 she may be OK because
the original should be in her case file.

The physician is to provide it. It does not have to filed with the I-
485 but should be taken to the interview.

Note that the vaccination supplement accompanying Form I-693 is not
included on this internet posting. The Centers for Disease Control and
Prevention (CDC) have sent the vaccination supplement directly to the
civil surgeons for them to photocopy, fill out on behalf of each
applicant, and attach to Form I-693 for submission to USCIS.

Tell the doctor to get on the ball. He can either contact the CDC or
the USCIS for the form.

Two separate things, JEff. The I-693 supplement is not the DS-3025 and
the DS-3025 does not have to be included with the I-485. The medical is
waived for the filing of the I-485 for K-1 recipients. However, at the
time of the interview they are usually told to bring the I-693
supplement and they can show the doctor the DS-3025 copy to him as proof
of what vaccinations they have already had.

Hi Sevros - the Civil Surgeon(s) you've talked to are lying - here's
what it says about the V.S. on the official USCIS website:

"Note that the vaccination supplement accompanying Form I-693 is not
included on this internet posting. The Centers for Disease Control and
Prevention (CDC) have sent the vaccination supplement directly to the
civil surgeons for them to photocopy, fill out on behalf of each
applicant, and attach to Form I-693 for submission to USCIS."

So there you have it - straight from the horse's mouth ;-)

The Vaccination Supplement *is* a document you get from an INS
designated Civil Surgeon. To get my VS - I took my vaccination details
along (from both US and UK) which showed which innoculations I'd had.

After a bit of arguing about whether I needed a full medical (he said I
did, I said I didn't as I'd already had one as I was on a K1) he
eventually agreed to give me the VS (for a fee) which was signed,
stamped and sealed in front of me. I left it all sealed up and just
included it with my AOS application.

Some doctors don't seem to like just giving you the VS - they will try
and get a full medical out of you (and subsequent large fee). Don't
take any crap though - your wife just needs the VS - NOT a full
medical. If they say they don't know where to get the VS and they are
an INS approved doc - they're lying.

We are only going to be charged $25. I faxed Honey's shot records to
the doctor's office when i made the appointment and they verified that
he has everything he needs, so they said that if i just bring in Honey's
passport and shot records, then he doesnt even have to take time off of
work to go in to the doctor's office.

not at all - I don't know if the fee is standard across all doctors - or
just the one I went to - but I paid $70.

Yikes, sounds like you paid a lot! I like Marnee's fee better. ;)

Well, the first doctor we called said $300 and included the full
medical whether we needed it or not, but we just kept dialing and came
across this one.
