Hi Sevros - the Civil Surgeon(s) you've talked to are lying - here's
what it says about the V.S. on the official USCIS website:
"Note that the vaccination supplement accompanying Form I-693 is not
included on this internet posting. The Centers for Disease Control and
Prevention (CDC) have sent the vaccination supplement directly to the
civil surgeons for them to photocopy, fill out on behalf of each
applicant, and attach to Form I-693 for submission to USCIS."

So there you have it - straight from the horse's mouth ;-)
The Vaccination Supplement *is* a document you get from an INS
designated Civil Surgeon. To get my VS - I took my vaccination details
along (from both US and UK) which showed which innoculations I'd had.
After a bit of arguing about whether I needed a full medical (he said I
did, I said I didn't as I'd already had one as I was on a K1) he
eventually agreed to give me the VS (for a fee) which was signed,
stamped and sealed in front of me. I left it all sealed up and just
included it with my AOS application.
Some doctors don't seem to like just giving you the VS - they will try
and get a full medical out of you (and subsequent large fee). Don't
take any crap though - your wife just needs the VS - NOT a full
medical. If they say they don't know where to get the VS and they are
an INS approved doc - they're lying.
