Default An updated DMP and the form 5419 available?

It seems that a directory of the designated medical practitioner in
the US is out of date. Lots of the DMP's phone numbers actually are
not reachable. Where can people find the updated one? Also, where can
people download the medical report form IMM 5419?

You'll get list of DMP's with your medicals request. Same with the form -
you cannot download medical forms, they will be provided to you pre-printed
with your file number, name and date of birth and with photo attached and
stamped. Wait for request, you can't do medicals until you get request and
forms. You will receive only section A and DMP will have the rest of
sections needed for medical report - you won't be allowed to even see them
nor review.

Only sponsored spouses and common-law or conjugal partners do their medicals
upfront.

If you are the sponsored spouse or common-law or conjugal partner then you
don't need to worry about any forms - just complete simple form page from
medicals instruction in your application kit and take it (with photos and
passport) to DMP with you. List of DMP's on CIC website is quite up to
date - maybe you can't reach particular doctor due to summer holidays?

Thanks for the detail explanation. I will check the CIC web site for
the updated DMP contact info. Having heard the medical examination is
usually required to complete within 90 days, does it mean the DMP also
must send out the medical results within 90 days? The applicant cannot
control the DMP's routine work. If the DMP delays sending out the
medical reports such as several weeks, will it result in the
application rejected or only get the processing delay?

In addition, is it necessary for the applicant to inform the visa
office about the date of the completion of the medicals? If so, how to
do it?

DMP handbook instructs DMP's to schedule an appointment with applicant
within 10 days from the call from applicant. All lab tests results are
usually back in DMP office within week or two and there is absolutely no
problem making it all within 90 days as long as you call DMP within month or
so from receipt of request for medicals. Note that if you applied through
Buffalo then and you are required to attend the interview then you will get
medical forms together with decision about need for interview and you'll be
advised not to do medicals until you receive interview date. Cool down and
relax...

So, it is unnecessary for the applicant to inform the visa office of
the medicals already done, correct?

Yes, it is not necessary as long as medicals were done using forms received
from visa post and results were submitted properly by DMP.

I also heard the expiration date of the immigration visa would be same
as the expiry of the medical report. So, the people can know the last
day of entering the Canada as soon as their medical examination is
completed unless the immigration visa is not issued within one year
after the medicals. Is it correct?
