Question re GC holder re-enter rules

What are the rules for a Permanent Resident (Green Card holder) re-
entering US?
If someone has been out of US for more than 6 months, would there be
an issue with immigration officials?

It depends. Every case is unique and they look at every case on an
individual basis. Have you maintained ties to the USA? Did you file US
taxes? Have you maintained your residence in the USA?

It's usually a good idea to get a re-entry permit before leaving the
USA, if you plan to be out longer than 6 months.

You bet there is an issue.

Keep your calm... if this is the first time you have been out for more than
6 months than it should not be that big of a problem. Just don't piss of
the officer and he will probably let u go. some will stamp your passport to
say that you were out longer than 6 months.
