Default Mr Miller - Family information disclosure in the application form

A) I am single and never married. My family is parents and
siblings. None of them accompany me to Canada. In my application for
permanent residency (skilled class worker), in which form do I need to
mention the information about my parents and siblings. In the the
application for permanent residency (Form IMM 008 Generic) or
additional family information (Form IMM 5406) or in the both?

Do I need the photographs for my parents and siblings (including who
are married) even though they do not accompany me to Canada?

In the guidance for filling up form IMM 008, it is mentioned that
family "includes" spouse, common law partners and children. It
doesnot specifically mention about parents and siblings.

B) In case of medical examinations, upon request from CHC, are my
parents and siblings also required to get the medical test done?

Just include their deetails in Additional Family Information Form. They
have no further part in your application.

You should provide information about your parents and siblings in IMM 5406
(Additional Family Information) and also about parents in your Schedule 1 -
sections 7 and 8. That it all, no photos, no medicals, nothing else for any
of them as they are not part of your application.
