On the medical for 26,

I know I have to put the "file reference & the ICSE request ID"
somewhere.

But where does it go on the form?

Also, where it asks "visa type" would I put "Class BN, subclass 136
skilled independant"

Oh, and does anyone have any idea where my man George Lombard has got
to?

Hi Jazzys,

I don't remember writing the file ref/ICSE on the form but I do remember
giving the details to the panel doctor and he wrote them on somewhere.

Yes you have got the visa type correct, thats what I put.

Jazzy you dont - you just put your passport number in the top right hand
corner. I have just double checked on the form - I was going to say I
didnt remember writing the case ref number on it.. I only had my meds
done last month so have the up to date forms. x

Hi Jazzy,

On the first page on the bottom left it says for office use only, but
according to the instructions that I have (I'm filling the forms in
also) that is where it tells you to put in the visa type and file
reference number. I have retrieved this information from the email that
was sent to me on Friday stating that we have case officer so is up to
date. Hope this helps

Our instructions didnt say to fill that bit in - glad you pointed it out
