Default I-485 - Nonimmigrant Visa

There is a couple of questions I don't have answers for on the I-485
form.

-Nonimmigrant Visa Number
-Date Visa was issued
-Consulate where
visa was issued

Will I automatically get rejected without a chance to
explain if I put N/A in these blanks? Or will everyone who files AOS get
a chance to present their case to an interviewer?

Edited to explain: My
last re-entry was in 1999 from a 4-day visit to Canada where I did apply
for an re-entry visa based on my H1B visa. At that time the Automatic
Visa Revalidation rule (implemented since April 2002) did not exist.
Even though my visa application was denied (missing copy of signed LCA)
I was waved through at the US border no problem. Now I don't what to put
for these I-485 questions. (Note: The only visa I had before my change
to H1B was my F-1 but that had expired in 1998.)

so, what's the problem? just indicate all the information about your F-1
visa. Many people come here on F-1 and then change their status to H1,
and they don't have H1 visa. So you just show them what you are asked
for, don't overdo it.

Many do people come here on F-1, change their status to H1 BUT they
don’t leave for a 4-day trip to Canada, is denied a H1B visa due to a
missing page of their application copy, and still are able to return to
the US because of their I-797A (not a visa) I guess! If I answer
truthfully my answers on I-485 (AOS due to marriage to USC) look
“weird" because I didn't have a valid visa at entry!!!

I am convinced I was allowed back in the country in 1999 (pre
9/11) due to the “Contiguous Terrritory� rule because no one game
me a hard time. Are you an attorney? If so do you think I am legal or
out of status?
