Hi Marie,

You will need to try and detail all the countries you have been to with
approximate dates.

Just put down what you can remember.

Good luck.

Hi Marie,

We had to do this last week. Thankfully my wife has a Polish passport so
there were stamps for everywhere we had been. Form 80 really has become
a pain. It took us ages to retreive our old passport details.

If you are looking for your old passport numbers you have to contact the
Disclosure of Information Dept of the Identity and Passport Service in
Peterborough requesting the old number in writing, prove your id (by
faxing a copy of your current passport) and send them a cheque for 10 uk
pounds. Address is:

Identity and Passport Service
Disclosure of Information,
PO Box 215
Peterborough
PE1 QQ.

They told me they were getting a lot of requests recently. I guess this
because the form 80 has changed.

I think as long as you can provide rough dates you'll be fine, I'm sure
they understand that people won't remember the exact dates for 10 years

That question was a total nightmare. I just put month and year approx
and had to use an A4 sheet as well (We like our holidays!). Dont think
we were totally correct give or take a month or so but just have a go
and put down as much as you know.
