Default Height/Weight details in Passport request

Hi Guys,
I got the PR request yesterday and took the photo specifications
letter with me to photo studio. Luckily, I detached my passport
request letter from that. By the time, I came out of the studio, some
one else took the photo specs form. I still think, it would be ok, but
I still need to send them the information like eye/hair colors,
weight, height. Can somebody tell me what all information, they ask on
that form. I can attach a letter with my mail describing all the
details and send it to them.

Hi Raj,

FYI, the request for height and eye color should be seperate
but not on the same photo specs sheet.

Anyway, this is what they asked
for me. This is for the principal applicant and dependents. Height and
eye color. Thats all. No hair color or weight.

You may just draw a
table with names in one column and details in following columns.

Hi ,

I had photocopied the sheet before I send to the consulate . if
you want the sheet pls let me know ur fax number and I can send it to
you or ur email address , I can scan and send you the sheet .

The only
thing they ask is Hieght and Eye color .
Yes the photo spec and this
sheet are on the same paper opp sides.
