I was born in Australia (Victoria), and have been living for many years
out of Australia.
My name today is different than the one given to me at
birth, but I have certified documents proving my connection to this
name.
I also have a photocopy of my birth certificate, but not the
original one.

My questions are:
(a) How do I get my original birth
certificate from the BMD (Birth, Marrieage and Death) Registration
office in Victoria state?
(b) What documents do I need to provide for
this purpose, bearing in mind that I live outside of Australia?
(d) What
is the exact address of the office I send my request to?
