Default Filling Out I-94

Hi all,

I am prepping a mock up of the I-94 for my Dad to follow when he comes
to visit later this year (he is a very nervous and infrequent
traveller).
On the I-94 document, #10 states "City Where Visa Was Issued".

If he lives in A in England, departs from B in England and lands in C
in the US, which do I have him fill out here?

(While here and asking, all I presume he needs to fill out for
visiting is the I-94 and bring his passport with him, correct? - he is
a born and bred Englishman)

Your question's a bit confusing. Most British citizens don't need a
visa to visit the USA; they enter using the Visa Waiver program and
fill out a green I-94W form rather than a white I-94. The I-94W form
doesn't ask about where the visa was issued since there is no visa
involved.

If your Dad is using a visa for some reason, then he just needs to
answer the question - put the name of the city where he got the visa
(which would almost certainly be London if he got the visa in England).

If he has an actual visa in his passport, the place where the visa was
issued from is written in that visa.
