Your question's a bit confusing. Most British citizens don't need a
visa to visit the USA; they enter using the Visa Waiver program and
fill out a green I-94W form rather than a white I-94. The I-94W form
doesn't ask about where the visa was issued since there is no visa
involved.
If your Dad is using a visa for some reason, then he just needs to
answer the question - put the name of the city where he got the visa
(which would almost certainly be London if he got the visa in England).
