Hi All

I changed my name by deed poll in the UK 10 years ago, My question is
how many Police Certificates do I need to get? I.E. - One for each
surname or do I just put my previous surname in the "previous surname"
box on the application form, what does the consular officer want to
see? I have all my paperwork in my new name including Passport and
Birth Certificate as I am adopted and I just changed back to my
Original Birth Name, I infact did not actually need to use the deed
poll as I am entitled to both names.

Any info appriciated.
