Default Applying for replacement of lost passport in US - Urgent Question

Hi,



I recently lost my passport and need to get a replacement within 6
weeks. I have almost completed the replacment form C1, however, there
are a couple of questions which are causing a few problems.



One of them asks "Have you ever lost a British passport before?". It
asks for the name of the person who signed to get the passport.



Now, the first passport I lost (before the most recent one) I lost
around 2 years ago, but I got it about 7 years ago and cannot remember
the name of the person who signed for it.



What should I put down? Should I tell them I can't remember the name?



I'm a little worried and any help would be greatly appreciated
