Default Adding new born to the application

I have applied for permanent residence in Canada in the Skilled
Workers/Professionals category. My interview was waived and the medical
documents were sent to me last month. I received my new born's birth
certificate last week and now I am working on getting his US passport. The
medical papers sent by CIC states that I must take my medical within 60
days.:

1. How do I add my two month old to my immigration application?
2. Since he need a medical exam as well, should I wait for his medical
papers before we take our (myself, my wife and our older son) medical exam?
