Dear Sir,

I have received the Medical Report: Section A Client Identification &
Summary for me and my wife on 7th April 2004 from CHC Islamabad office
along with a paper that lists the name and address of DMP.

Today we went to do our Medical Exam, where the hospital
administrative staff asked us to give the mailing address where the
DMP should send the Medical Exam results. The medical forms that I
have received from Islamabad Visa office, do not mention this mailing
address.

Kindly advise me asap what should I do or otherwise tell me the
mailing address?

Does the DMP himslef know where to send the medical results?

FYI, I previously had immigration medical exam also during Feb 2001
when my file was in CHC office in Buffal but at that time I could not
go for interview so I got my file transferred to CHC Islamabad.At that
time alongwith my Medical Papers: Section A Client identification &
Summary I received Instructions for DMP from Buffalo office and they
asked the DMP to mail the reports to the following address:

Canada

Please advise.
